Knapp’s Development “My Shopping Choice” Order Process
1) IC or Participant visits www.myshoppingchoice.com and downloads Purchase Request Form
2) IC or Participant fills out Purchase Request Form
2a) Individual filling out form must provide as much information as possible including SKU #’s when available, screen shots of items, complete description of the items including size and color when applicable and any other pertinent information relating to the order.
2b) Knapp’s Development does not make any purchase decisions for the member or IC. All information related to the order must be determined by the member and IC.
3) IC emails completed Purchase Request Form to order@myshoppingchoice.com
4) Knapp’s Development confirms items/prices and updates the Purchase Request Form to reflect all costs associated with the order including shipping costs, sales tax and admin fee (15% of the cost of the item with a minimum fee of $95.00 per order).
5) Knapp’s Development emails IC updated Purchase Request Form
5a) IC adds Knapp’s Development as a vendor on the Members Plan.
5b) IC adds item/s from the order to the Members Plan with one authorization
5b1) Auth = Item/s, Shipping, Sales Tax and Admin Fee
5b2) IC must include a note of what item(s) are being ordered in order for the claim to be paid
6) IC or FEA sends Auth Letter to Knapp’s Development
6a) Knapp’s Development confirms Auth is correct
6b) Knapp’s Development requests Member ID # and Date of Birth
7) Knapp’s Development places order and notifies IC and/or Member of expected delivery date/s
8) Order is delivered
9) Knapp’s Development confirms the correct item/s were received with no damage
10) Knapp’s Development bills members FEA
